What this class is

GLP-1 receptor agonists are injectable medicines that mimic a gut hormone involved in appetite, stomach emptying and blood sugar control. They were developed for type 2 diabetes and some are registered for weight management. They are real medicines with real effects, which is the whole point: something that changes appetite and gastric emptying that much also has side effects and contraindications.

Why they are script-only here

In South Africa these are scheduled medicines. They are supplied against a valid prescription through a registered pharmacy — that is not pharmacy gatekeeping, it is the law and it is also the safety mechanism. Someone has to check your history, your other medicines, your kidneys, your thyroid and pancreas history, and what happens when you stop.

We do not discuss stock or price of scheduled medicines on a website, and you should be sceptical of anyone who does.

The grey market problem

Compounded vials, powders to reconstitute, unlabelled pens, and product bought through social media all share the same three unknowns: what is actually in it, what dose you are drawing up, and whether it stayed cold from the factory to your fridge. Cold chain is not a technicality — a peptide that has been warm in a courier van is not the medicine on the label.

Add the injecting itself: non-sterile technique, reused needles and shared vials cause abscesses and worse. We see the consequences, not the marketing.

The things people are not told

Muscle: rapid weight loss without protein and resistance training takes lean mass with it. That is a bad trade at any age and a worse one after fifty.

Gut: nausea, vomiting, constipation and, less commonly, serious pancreatic and gallbladder problems. Severe abdominal pain is a same-day emergency, not a side effect to push through.

Rebound: appetite generally returns when the medicine stops. Without a plan for food, training and follow-up, so does the weight.

Contraception: delayed stomach emptying can affect absorption of oral medicines, and pregnancy is not the time for this class. If you are on the pill and considering this, that conversation has to happen up front.

When to stop self-managing

Severe or persistent abdominal pain, repeated vomiting, signs of dehydration, jaundice, or a lump, redness or fever at an injection site — get seen the same day.

If you are considering a GLP-1, start with a doctor's consult and bring the plan to us. If you already have a parcel in the fridge and are unsure what it is, bring it in before you inject it. We will be straight with you and we will not lecture.
